What is a Good Faith Estimate?
A Good Faith Estimate shows the expected cost of scheduled therapy services. It is provided in writing and includes the total expected cost of items or services related to your care, including any related costs such as diagnosis, testing, or other services.
When will I receive one?
If you are uninsured or choosing not to use insurance for your care, a Good Faith Estimate will be provided at least 1 business day before your scheduled service. You may also request one at any time.
What is included?
A personalized estimate will include the fees associated with the services you are expected to receive. Because therapy is individualized, the frequency and duration of sessions can vary. Your estimate will reflect the fees for the services discussed at the time it is prepared.
How to request an estimate
You can request a Good Faith Estimate by emailing jamieruthlpcc@outlook.com or by requesting one when you schedule your appointment. We are happy to answer questions about fees, insurance, and payment before you begin care.
If your bill is higher than your estimate
If you receive a bill that is at least $400 more than your Good Faith Estimate, you may dispute the charges through the patient-provider dispute resolution process. For more information, visit cms.gov/nosurprises.
