Complaints
If you believe your privacy rights have been violated, you may file a complaint with us using the contact information above. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
Changes to this notice
We reserve the right to change this notice and the privacy practices described in it. Any changes will apply to the information we already maintain, as well as information we receive in the future. The revised notice will be posted on this website and made available upon request.
