HIPAA Notice of Privacy Practices

Your protected health information is kept private and secure. This notice explains your rights and how your information may be used.

This notice is maintained by Jamie Ruth, LPCC to describe how your protected health information (PHI) may be used and disclosed. It also explains your rights under HIPAA. Please review it carefully and keep a copy for your records.

Our commitment to your privacy

We are required by law to maintain the privacy of your protected health information, to provide you with this notice of our legal duties and privacy practices, and to follow the terms of this notice currently in effect.

How we may use and disclose your PHI

Your protected health information may be used and disclosed for the following purposes without your written authorization:

Treatment: To provide, coordinate, or manage your care and related services.
Payment: To obtain payment for services provided, including from your insurance plan or a third-party payer.
Health care operations: To support the business activities of the practice, such as quality improvement, training, and compliance.

We may also use or disclose your PHI without authorization in situations such as:

To prevent or avoid serious harm
For public health activities
For health oversight activities
In response to a court or administrative order, subpoena, or other lawful process

Your rights regarding your PHI

You have the right to:

Request restrictions on certain uses and disclosures of your PHI
Receive confidential communications by alternative means or at alternative locations
Inspect and obtain a copy of your PHI
Request an amendment to your PHI
Receive an accounting of certain disclosures of your PHI
Receive a paper copy of this notice upon request

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us using the contact information above. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.

Changes to this notice

We reserve the right to change this notice and the privacy practices described in it. Any changes will apply to the information we already maintain, as well as information we receive in the future. The revised notice will be posted on this website and made available upon request.
